Cooper Test
The Cooper test measures the greatest distance a person can run in 12 minutes. The result estimates VO2max with the published formula VO2max = (distance in metres - 504.9) / 44.73. It needs only a track, a stopwatch and a maximal effort.
Cooper Test: what is it?
Kenneth Cooper developed the 12-minute run in the 1960s for the United States Air Force, needing a way to assess the aerobic fitness of large numbers of people without a laboratory. That constraint is exactly why the test still survives: it works on any measured loop, it tests a whole group at once, and its output converts directly into a VO2max estimate. The catch is that it is a maximal effort, which makes pacing skill part of what it measures - and makes it unsuitable for clients who are deconditioned or unscreened.
The protocol
Run or walk as far as possible in 12 minutes on a flat, measured course - a 400 m athletics track is ideal because you can count laps and read the final part-lap off the markings. Warm up for 10 to 15 minutes with easy jogging and a few strides. Start the clock and go. Record the total distance in metres at the 12-minute mark.
The instruction to the client is to cover the maximum distance, not to run a comfortable 12 minutes. Almost every first-time tester goes out too fast and walks the last three minutes, which understates their fitness. Tell them the target is an even pace they can just hold for the full duration, and call out the split at 6 minutes so they can adjust.

The formula

The equation is a regression fitted to a population, so it carries real error for any individual, and it assumes a genuinely maximal, well-paced effort. Two things follow. Report the result as an approximate figure rather than a precise one, and put more weight on the raw distance than on the converted number when comparing a client to their own previous test.

Who it suits and who it does not
The Cooper test is a good fit for healthy, active clients who can already run continuously for 12 minutes, for team-sport groups where you need to test everyone in one session, and for anyone preparing for a fitness entrance test that uses it - many military, police and fire services still do.
It is a poor fit for beginners, for clients returning from injury, and for anyone unscreened, because a maximal effort in an untrained person is both risky and uninformative. For those clients, a submaximal walking test or a talk-test-guided assessment gives you more usable information with none of the risk.
Reading the retest
The value of the Cooper test is almost entirely in the comparison. Same course, same conditions, same time of day, eight to twelve weeks apart. A meaningful improvement shows up as extra distance - a hundred metres or more is a real change; twenty metres is within the noise of pacing and weather.
Record the conditions alongside the result. Wind, heat and surface matter enough that a result run into a headwind on a hot afternoon is not comparable to one run on a calm cool morning, and a client who is not told that will read a slower retest as failure.
